Catwalkfever.com will keep confidential and never publish a model's last name, street address or phone number. E-mail and website addresses will be posted online only if applicant chooses to have this information made public on the entry form. The exception is for models under the age of 18, whose e-mail and website address will never be published online and will be kept confidential. </div> <bR> <div> <B>Materials</B><B> </B></div> <div> All photographs and other materials submitted become the property of Catwalkfever.com, and as such will not be returned. By submitting materials to Catwalkfever.com, entrants give permission to use these materials for promotional purposes related to Catwalkfever.com, its events and operations, at its sole discretion without any further compensation or notification. </div> <bR> <div> <B>Communication with Catwalkfever.com</B><FONT SIZE="3" COLOR="#FFFFCC" FACE="Arial" > </FONT></div> <div> Entrants will only be contacted (phone, e-mail or mail) if they are chosen to become a model for Catwalkfever.com. If you are not contacted, it is because your entry was not chosen by our panel to become a model. Entrants may be considered at a later date and may be contacted to work with one of our photographers or consultants to further their chances in becoming a model on Catwalkfever.com. </div> <bR> <div> <B>License</B><B> </B></div> <div> I hereby grant to Catwalkfever.com, the absolute right and permission to use and reproduce the photographs, copies of which are annexed hereto ("Photograph"), either alone or accompanied by other material, in any manner and in any media, throughout the world, at any time hereafter, for any purpose in connection with the business of Licensee, primarily in interactive media, on a worldwide basis. </div> <bR> <div> I hereby warrant and represent that I am the sole and exclusive owner to all rights to the image in the Photograph including all copyrights, that I am 18 years of age or over, and that I have the full power and authority to enter into this agreement and to grant the rights granted herein. I further warrant and represent that the use or reproduction of the Photograph for advertising or trade purposes, including, without limitation, such use in any interactive media or on the Internet, will not violate any law or the legal equitable rights of any person, firm or corporation.</div> <bR> <div> I agree that I will not hold Licensees or anyone who receives permission from any of them, responsible for any liability resulting from the use and/or reproduction of the Photograph, in accordance with the terms hereof, including, without limitation, for copyright infringement and/or what might be deemed to be any alteration or modification of the Photograph due to distortion, optical illusion or faulty reproduction which may occur in the finished product.</div> <div>  </div> <div> I understand and acknowledge that nothing herein will constitute any obligation on the Licensee to make any use of any of the Photograph and/or rights set forth herein. I hereby waive all my rights to inspect and approve the finished product, its use, or such written or spoken copy as may be used in connection therewith. I warrant and represent that this license does not in any way conflict with any existing commitments on my part.</div> <bR> <div> I understand and acknowledge that this Agreement represents the entire understanding between the parties and that this Agreement cannot be modified or altered, except in writing, signed by the parties. I understand that any dispute and/or disagreement arising under or in connection with this agreement shall be governed by the laws of the State and shall be fully and finally decided by a court of competent jurisdiction in that State. </div> <bR> <div> <B>Agreement</B><FONT SIZE="4" COLOR="#FFFFCC" FACE="Arial" ><B> </B></FONT></div> <div> By submitting the modeling form, I hereby understand and accept the rules, terms and conditions outlined above, and verify that all information provided on the Entry Form contains no false information. </div> <bR> <div> <B>Parental Agreement</B><B> </B></div> <div> Entrants who are currently 17 years old or younger must have a parent or legal guardian fill out the information and sign below. I, as parent/legal guardian of the model, agree to the above, and personally join in the warranties and representation set forth above. I sign this document to signify my agreement, and I agree to indemnify and hold harmless the Licensees with respect to any claims which the minor may make as a result of the exercise by the Licensee and/or Sublicenses of their rights hereunder. </div> <bR> </td> </tr></table></body>
